You can use the plan as much as you like and pay discounted plan prices
for all services you require.
- ORTHODONTICS (braces) INCLUDED
except orthodontic treatment in progress. For both children AND adults,
at a 20% to 60% reduction on usual and customary fees.
- COSMETIC DENTISTRY INCLUDED.
Examples: bleaching or aesthetic bonded finishes in selected markets at
a 25% reduction on usual and customary fees.
- DENTAL IMPLANTS INCLUDED.
Dental implant services in selected markets at a 25% reduction on usual
and customary fees. Practitioner's cost of implant fixture excluded.
- ALL SPECIALISTS INCLUDED.
Periodontists, Endodontists, Oral Surgeons, Orthodontists and
Prosthodontists (25% reduction on usual and customary fees where
available).
- ALL ONGOING DENTAL PROBLEMS ARE
ACCEPTED. The only exception is for orthodontic treatment in
progress. Otherwise, even if you have had a dental problem for a long
time, your condition is accepted under this Plan.
- NO DEDUCTIBLE. Your
savings start with your first visit. You know exactly how much you must
pay out of your own pocket each time you see a dentist.
- NO AGE LIMIT.
Regardless of age, you take advantage of Dental, Vision,
Prescription and Chiropractic Care benefits.
- NO CLAIM FORMS. No
insurance companies and no unexpected payments to deal with.
- ACTIVATION WITHIN ONE
BUSINESS DAY. You can go to the dentist once your
application has been processed. It takes one business day to process an
application if you Sign Up online or by phone using a credit card or a
checking account.
- CHANGE DENTISTS ANYTIME.
You can change dentists whenever you want without having to notify anyone.
- MEMBERSHIP FEE GUARANTEED for two
years if you sign-up today.

If you are not satisfied within the first 30 days of becoming a member,
you may cancel your membership in writing by mail to 5700 Democracy
Drive, Plano, Texas 75024 or fax to (469) 229-4595. Your membership fee
will be refunded. Please be aware that the one-time administrative fee
and money paid for healthcare services and products are not refundable.
After 30 days membership fees are not-refundable. You may cancel your
membership at any time upon written request to AmeriPlan®.
